VOLUNTEER FORM INSTRUCTIONS

 

The Fresno Dixieland Society - Sounds of Mardi Gras 2020 will be held at the DoubleTree in Fresno on Feb 6-7-8-9, 2020.  The festival is not far off, and as always, we will need volunteers.  We prefer you submit your application electronically.  The application can also be filled, printed and mailed in.  We will need volunteers in all the usual positions:  door monitors, badge sales, band and hospitality  rooms.  No shifts are available on Thursday.  In exchange for volunteering, you will receive a same day pass for working 4 hrs on Friday  or 4 hrs on Saturday or 2 hrs on Sunday.  You will be given a DAY PASS for each day you work your assigned hours.  You can work as many days as you like but this depends upon shift availability.

 

The green VOLUNTEER APPLICATION button beloW will take you directly to the VOLUNTEER APPLICATION FORM.  Please fill in all of the requested information.  We count on you to be there for your assignment.  Please do not volunteer unless you are certain you will be able to fulfill your assignment.  Volunteer passes do not include:  Thursday Pre-Dance, Band Meet and Greet, Wind Down Dinner or Wind Down Dance.

 

 

 

 

 

INSTRUCTIONS FOR COMPLETING FORM

 

Volunteer Information:  Please fill in the blanks.  Name and phone are required.  Please include an eMail address if you have one.  We prefer to send you your assignment by eMail but will phone you if necessary. 

 

Number of Volunteer Days:  Indicate how many days you would like to work.  Select "1,2,3 or as many as needed".  If you request more than one day, you will be assigned multiple days only if available. 

 

Volunteer Shift Availability:  Check all boxes that you are available to work.  You may only be assigned to work on multiple days if you also indicated more than one shift under NUMBER OF VOLUNTEER DAYS.

 

Name of Partner Volunteer:  If you are pairing up with someone such as your spouse, girlfriend, boyfriend and want to be scheduled during the same time or block, please provide the name of that person.  We will do our best to keep you together as much as possible.  VERY IMPORTANT:  Each of you must submit a separate Volunteer Application and include each others name.  

 

Special Limitations:  If you have special needs or limitations such as a walker, wheelchair, limitations to stand or walk for prolonged periods of time, please let us know.  We want to assign you, whenever possible, to an area that is comfortable for you.  

 

In Case of Emergency:  Please provide the name and info of someone to contact in case of an emergency.  In case of an emergency, we will phone, text and eMail that person.  

 

Volunteer Agreement:  Please confirm that you agree to accept your assignment based on this application.  You are agreeing to fulfill your assignment in exchange for a day pass.
